SUBMITTING ADVISING FORMS ELECTRONICALLY

General Policy + Procedure

Advising forms help you plan your classes for the upcoming term and verify that you know which courses you are required to take. If you fail to complete and submit your advising form by the deadline, you will have an Advising Hold placed on your account and may be unable to register on time. Advising forms do not reserve seats in classes. You must manually register online.

Electronic Submission of Advising Forms

Since all CADA Student Affairs staff are currently working remotely and will not return to campus for the foreseeable future, you will need to submit your advising form electronically if you did not submit it prior to Saturday, March 14th. Please follow the guidelines below to ensure that your form is legible and processed in a timely manner.

Scanner (Preferred)

Scan your form using a scanner and save it as a PDF. Attach the PDF to an email to mrmk@uic.edu.

Use an app like CamScanner (there are many free options for both iOS and Android) to save your form as a PDF. Crop the PDF to the form if necessary. Attach the PDF to an email to mrmk@uic.edu. Photo Take a photo of your advising form. Make sure there is good lighting, limited or no shadow, and the picture is in focus with legible responses. Crop the photo to the form if you have excessive background space. Attach the photo to an email to mrmk@uic.edu.

Fill electronically

Fill out the PDF electronically using Adobe Acrobat or another program. Make sure your added text felds with your responses save to the fle and are visible when opened in another program or on another machine. Attach the PDF to an email to mrmk@uic.edu.
